HMO vs PPO

An HMO usually costs less and uses a primary doctor as the gatekeeper. A PPO costs more and lets people see specialists and out-of-network doctors without a referral.

Neither is automatically better. It depends on whether your people will stay in-network and how much premium the company can stand.

Level funding and reference-based pricing

Fully insured: you pay a fixed premium. The carrier keeps the risk and any leftover.

Level funded: you still pay a set monthly amount. Unused claims money can come back. Stop-loss caps a bad year. Often a fit in the 25–200 employee range.

Reference-based pricing: the plan pays a benchmark (often a percentage of Medicare) instead of a PPO “discount” off a sticker price. Savings can be real. Employees need a clear story so a hospital bill does not land on your desk.

401(k) and CalSavers

California employers with five or more employees who do not already sponsor a retirement plan must offer a plan or join CalSavers.

A 401(k) is usually the better long-term answer if you want matching, payroll sync, and real plan design. CalSavers is the state default if you do nothing.

What is a SHOP plan?

SHOP is the small-group marketplace. In California that runs through Covered California for Small Business. Same essential benefits as other ACA small-group plans. For some groups under 25 it is the path to the small-business tax credit.

You do not have to buy SHOP to offer group coverage. It is one shelf we shop, not the only shelf.
